Often, medical rehabilitation services are the next step in medical care, especially if your family member has been in the hospital for an acute illness, surgery, or significant injury. Rehabilitation medical services help people in recovering some or all of their mobility and functioning lost due to their condition or treatment.
For example, a family member recovering from major surgery, like a hip or knee replacement, will need therapeutic exercises and coaching to be able to regain functional mobility. Your loved one may still be feeling the effects of hospitalization and needs some form of medical attention for a period of time. At the Illuminati Care Community, our certified skilled nursing facility utilizes the household model of care within our neighborhood levels. We strive to create a real community within our 23 households that our residents recognize as home. Personal choice, the natural rhythms of the day, and meaningful life all make up the household philosophy to ensure that elders live in a state of well-being.
The physical environment is warm, accessible, and residential in nature. Our households are limited to 8 to 16 residents. This gives our residents and staff alike a sense of ownership and is key to a sense of control. Residents are given direct access to the kitchen, as well as comfortable shared spaces – and this nurtures relationships and creates a sense of home.

- 24-hour RN supervisory coverage, 7 days a week
